General Health District Birth and Death Certificates

Certified Records

All birth and death records are maintained in the county where the birth or death occurs. If the birth or death occurred at home or at Clermont Mercy Hospital the certificate will be on file with Clermont County.  We recommend you contact us before arriving to verify that the certificate is on file with Clermont County. Since there is no birthing hospital in Clermont County most of the births in our area occur outside Clermont County.  Use the following hospital list to determine which Health District the certificate is filed with.

 

To obtain a copy of a certified birth or death certificate you may also print the certificate request form and mail it with a SASE (Self Addressed Stamped Envelope), and a check made payable to the Clermont County Treasurer to:

Genealogy Research

We have dedicated a computer workstation to genealogy research. All of our birth and death records are scanned into an imaging program so that the researcher is able to view an image of the actual record. As of July 1, 2003 in accordance with House Bill 95 uncertified copies will no longer be issued.  If a copy of a birth or death certificate is required a certified copy must be purchased.  The workstation is located at our Bauer Road office and the public is free to do research MTWHF 8:30-4:30.
