Birth/Death Certificate Request Form

Beginning January 1, 2011, Clermont County residents can obtain certified copies of Ohio birth records at their local health department.

In 2006 the Ohio Department of Health began scanning over 20 million birth records into a statewide database.  The completion of this project allows all local health departments the ability to issue certified birth records for any birth occurring in the State of Ohio from 1908 to present.

In the past obtaining a certified copy of a birth certificate was not so easy.  Birth certificates are filed in the county where the birth occurs.  Since there are no birthing hospitals in Clermont County this meant that most Clermont County residents had to travel to the City of Cincinnati, Hamilton County or even further to pick up a certified birth certificate.  “Now that all State of Ohio birth records are offered here at the Clermont County Health District it will be a great convenience for Clermont County Residents.” said Clermont County Vital Statistics Registrar, Melissa Jones.
